Questionnaires

To help us prepare to provide you free confidential SCORE counseling please complete the appropriate pre-counseling questionnaire below and, if applicable, the summary Business Plan form. Please remember to print a copy of each questionaire you use for your records before submitting it and bring it with you to your counseling sessions with SCORE.

If you already have a business plan please bring an updated copy of your current plan to all counseling sessions. SCORE provides Free Confidential Business Counseling, On-going Coaching/Mentoring, Free or Low Cost Small Business Training Workshops and Free Business Planning Materials.
